PriorityHRA plans

Our small business PriorityHRASM plans offer greater flexibility and funding options for employers.  Depending on the plan, you can choose the HRA reimbursement option: Employee pays first, HRA pays first, or HRA and employee split 50/50. Learn more about reimbursement.

Once the employer contribution is used up on the deductible, the employee is responsible for all coinsurance and copayments until they meet the out-of-pocket maximum. Even if copayments occur, before deductible, the employee must pay them, too.

PPO plans

PriorityPPOSM offers employees the greatest freedom of choice by providing in-network and out-of-network options. Employers reap the benefits of a low-cost health care plan, while employees can choose to see Priority Health network providers and pay less or visit non-network doctors or hospitals and pay more.

    Deductible:
    $2,000 individual, $4,000 family

    Coinsurance:
    Employee pays 30%

    Out-of-pocket limit: $7,350 individual, $14,700 family

    Primary / specialist / urgent care:
    $30 / $45 / $75

    Deductible:
    $5,000 individual, $10,000 family

    Coinsurance:
    Employee pays 30%
