I have a narrow network plan, what does that mean?

Your MyPriority® Beaumont Health Narrow Network plan offers one of our lowest monthly premiums and gives you access to quality care within the Beaumont Health system of doctors and hospitals.

Because this is a narrow network plan, you are required to receive care in the Beaumont Health system of doctors and hospitals and their affiliated clinics, outpatient facilities, labs, etc. Care received outside of the Beaumont Health network will not be covered, and members will be required to cover the full cost for out-of-network care.

Where do I find member forms?

Forms marked * are interactive, so you can type information right into them.

You can also save the completed forms to your computer, print them and mail them to Priority Health at the address below.

How do I pay my bill?

As a MyPriority member, you have the option to save time and money by enrolling in automated billing through your MyHealth account. We’ll withdraw your payment from your checking or savings account on the first business day of the month.

If you prefer to pay by check over the phone, you can call 800.528.8762 and our customer service team will help you.

How do I change my PCP?

If you ever need to change your primary care provider (PCP), you can do it a few different ways.

Through your MyHealth account

  • Log in to or register your MyHealth account.
  • Use our Find a Doctor tool to search for an in-network PCP or check that your preferred PCP is in your network.
  • Inside MyHealth select Insurance, then Change Primary Care Provider.

Fax or mail us the change PCP form

How do I create a MyHealth account?

We’ve made creating a MyHealth account simple for everyone.

  • Go to the MyHealth welcome page.
  • Click the “Register now” button and follow the instructions to create your account. You'll be asked to provide some basic information, like your name, email address and member ID number during the account creation process, so make sure you have your membership card ready.
  • You will also be asked to verify specific information about yourself so we can confirm your identity. These questions are similar to questions you'd answer when creating an account with your bank.
  • All family members on your Priority Health plan can have their own MyHealth account. Each person will have to create their own with a different email address.  

How do I set up an HSA?

If your membership card says “HSA” on the front of it, this means you’re eligible to start a Health Savings Account. You are required to pay 100% of the cost of your health care out-of-pocket until you meet your deductible.

Setting up an HSA can help you save money. Tax-free, to cover medical expenses. Plus, all MyPriority HSA-eligible plans include a free savings account through our banking partner, HealthEquity®.

  • Anyone can contribute money into your HSA each year.
  • Funds in your HSA may earn tax-free interest, so even if you're not contributing to your account, your HSA may continue to grow.
  • If you change health plans and are no longer eligible for an HSA, different rules will apply, but the money remains yours and you can use it for qualified health care expenses.

How do I order more membership cards?

If you ever need to order replacement membership ID cards, follow these steps below:

  1. Log in or register your MyHealth account
  2. Select the “ID Card” tile inside MyHealth
  3. Click the “Order ID Cards” button

Your new cards will be mailed to you at your home address. It can take up to two weeks for your new set of membership cards to arrive.
