If you’re looking to enroll in a MyPriority® individual or family health plan directly through Priority Health please delay your enrollment until Monday, July 29 or follow the instructions below. You can also call one of our enrollment specialists at 833.489.5443 for enrollment assistance.
- Download our Special Enrollment Period (SEP) application.
- Fill it out and sign it
- Once complete please email it to mypriority@priorityhealth.com. We will apply the effective date you indicate on the application, based on your SEP.
If you’re looking to enroll in a MyPriority plan on the Healthcare Insurance Marketplace please proceed directly to healthcare.gov.
If you’re looking to enroll in a MyPriority short-term plan please call one of our enrollment specialists at 833.489.5443 or follow the instructions below.

- Fill it out and sign it.
