Individual combined deductible
Your health savings account (HSA) works as a preferred provider organization (PPO) health plan. This means that medical services covered by your health plan are paid at either an in-network or out-of-network cost level, depending where you go. See your insurance policy for details. "Combined" refers to in-network and out-of-network costs.
You pay 100% of the cost of covered services until your costs reach the dollar amount of your Individual Combined Deductible. Then your plan begins to pay benefits.
- All services apply toward the Combined Deductible except for in-network preventive care (see Preventive Health Care Guidelines for details), which is usually a $0 cost to you.
- This deductible must be met each plan year.
- Once you've met your Individual Combined Deductible, your plan begins to pay for covered services.
