The Annual Notice of Changes informs you of any changes to the benefits to your plan for the next year. The Evidence of Coverage is the document you should reference in the upcoming year when seeking information about your coverage.
New for 2018!
As a Priority Health Michigan Public School Employee Retirement System Medicare member, beginning January 1, 2018, you’ll have access to the Silver&Fit Exercise and Healthy Aging Program. Join a local participating fitness facility or get up to two kits for exercising at home–whichever works best for you. Best of all, this program is included with your Priority Health Medicare Advantage plan and is available at no additional cost to you.

For 2018 we’ll be partnering with TruHearing to provide your routine hearing benefits, including hearing aids & a routine hearing exam, resulting in changes to your hearing plan and to the network of hearing providers.
Starting January 1, 2018, you’ll need to make sure your hearing provider is in this new Priority Health Hearing network or select a new hearing provider in the network in order to use your benefits.
To take advantage of your hearing benefit, call 855.205.6382 Monday through Friday from 8 a.m. to 8 p.m, (TTY users should call 711). A hearing consultant will verify your coverage and help you to set up a routine hearing exam with an in-network provider.
Please refer to your Hearing Certificate of Coverage document for details on changes to your hearing benefit.
The Drug List tells you what Part D prescription drugs are covered by the plan in 2018. It also lets you know if there are any rules that restrict coverage for a drug.

Provider and Pharmacy Directory
The Priority Health Medicare Provider and Pharmacy directory provides a list of Priority Heath Medicare's network providers broken out into five regions of the state. Some network providers may have been added or removed from our network after these directories were created. To get the most up-to-date information about Priority Health Medicare network providers use our online Find a Doctor directory or call Customer Service.
